The Iowa Program to Provide Coordinated Services for Children Who Need Technical Medical Care in Their Home/Community: A Network Approach.

The paper describes the Iowa Plan, a network approach to providing home care services for children with handicaps and chronic diseases who require medical technology services. The Iowa Regional Child Health Center Network is reviewed. The centers are based on shared responsibility between major state agencies and the community and serve as focal points for a network of regional services. Three elements are seen to be essential to the system: (1) professional services provided by physicians and pediatric nurse practitioners; (2) administration and community organization; and (3) parent support and advocacy. Action steps for the transition of services from the hospital to the community are delineated. The program is explained to be cost effective because only one new position--the home care program coordinator--is required. (CL)
