Keeping Track: Tracking Systems for High Risk Infants and Young Children.

Representatives of several states met to identify basic elements of a tracking system designed to ensure that disabled and at-risk infants and their families receive continuous and appropriate services to improve health and developmental outcome. Discussants explored six clusters of questions concerning the following: realistic goals and rationale; criteria of client inclusion and discharge, including related ethical issues and range of risk categories; instruments and techniques needed for optimal tracking and linkage to services; awarding primary responsibility for the system and what effect this choice will have on the terminology used; and parent role. Experiences with tracking systems in Washington, Maryland, North Carolina, Utah, Iowa, and Texas contributed to the discussion. Materials from those six states comprise the appendices, which are the main content of the document, and include forms for recording data on children, program comparisons, program descriptions, and program guidelines and procedures. The Iowa High Risk Infant Followup Program is represented by a comprehensive medical form. North Carolina guidelines and procedures with sample forms are included virtually in their entirety as a reference for those interested in reviewing how one state-wide tracking system functions. (RH)
