This survey collected extensive data from 770 county case managers, case manager supervisors, consumers, and service providers in Minnesota in order to provide a comprehensive description of case management practices serving individuals with developmental disabilities. The survey report describes current case management practices, identifies barriers to and gaps in services, and examines perceptions of the effectiveness of case management services. Following a review of the literature on case management, the survey methodology is presented. Analysis of survey results is organized by categories of the target groups: directors of county welfare and human service agencies, case management supervisors, case managers, consumers, service providers, school personnel, rehabilitation counselors, advocates, and public health nurses. An overall summary of results discusses: education and background of human services personnel; staffing patterns; case load; case management functions; case management time allocation; cooperative work among agencies; evaluation; barriers to delivery of effective services; gaps and duplication in services; perceptions of case management effectiveness in schools, public health, and county agencies; factors and strategies that contribute to effective case management; and recommendations. Samples of the nine case management survey forms sent to the targeted groups are included in an appendix. (JDD)
