Transition Strategies That Work. Volume II. Profiles of Community Collaboration.

Twenty-two transition programs illustrating effective community collaboration for youth with disabilities in Minnesota are profiled. These programs were selected because they applied a set of 10 quality standards developed to improve the transition process and adult outcomes for these youth, as well as to increase the cost effectiveness and efficiency of transition services and planning. Each profile includes information on: (1) participant disabilities, (2) ages served, (3) areas of collaboration, and (4) groups collaborating. Areas of collaboration included, among others: information sharing, staff sharing, co-funding, joint training plans, joint transition planning, shared public relations activities, shared resource library, program sharing, and assessment sharing. Descriptive information for each program includes an overview, a discussion of the need for the program, how the program meets the need, results and effectiveness data, and specific groups collaborating. Program address, telephone number, and name of a contact person are also provided. (DB)
