A National Health Insurance (NHI) bill is discussed. Such a law would address primarly two issues: financial coverage for catastrophic illness and some broadening of entitlements for ambulatory care. Severe discrimination against women in the medical industry is reviewed, including barriers to female appointments in prestigious positions, antagonistic atmosphere in the learning environment, frequent misconception about the career attachment of females, and nonsupportive environment for female staff in teaching hospitals. An interpretation of the dollar dimension of medical progress, the role of nonmonetary factors in the equation, and suggestions for public policy are presented. Medical leadership must help formulate recommendations aimed at reductions in medical school enrollments, fee schedules for physicians, costly technology, etc. (MLW)
